PATIENT CARE COORDINATOR

The medical office of Michael A. Randolph, MD PC seeks a full-time Patient Care Coordinator to provide clinical assistance to medical care providers while completing tasks and projects related to patient engagement, communications, and wellness. The Patient Care Coordinator is a combined role, performing both administrative and clinical functions for a medical office. The selected candidate will support patients’ needs during their appointments and beyond. Through their coordination of patient care, they will effectively bridge the administrative and clinical aspects of primary care.

The Patient Care Coordinator will report to the practice’s Practice Administrator. The position carries an expectation of a minimum of 32 to 40 hours/week needed to complete appropriate responsibilities

Requirements:

  • Minimum of a bachelor’s degree, preference in areas associated with medicine and/or health care
  • 1-2 years of experience providing medical care to patients or equivalent educational experience preferred
  • Familiarity with medical office equipment and patient care technology/systems
  • Proficiency with computers, including: Microsoft Office (Word, Excel, etc.), Google Drive

General Expectations/Responsibilities for the Position:

  1. Communications/Administrative/Patient Coordination Functions (75%)  
    • Create, update, and follow up with patient Care Management Plans developed with guidance from clinical providers
    • Speak with patients and family members over the phone as needed to ensure patients’ care and wellness needs are being appropriately accommodated
    • Ensure specialist referrals are appropriately coordinated, processed and documented through Electronic Health Record (EHR) including written and verbal facilitation of patient records per specialist request
    • Process medication authorizations and medication refill requests with assistance from health care providers
    • Correspond with patients in Patient Portal (HIPAA-secured email) to facilitate patient needs
    • Coordinate communication and care for Transition Care Management (TCM) protocol between providers, patients, families, and other healthcare team members including hospitalists or nurses when a patient is in the hospital or skilled nursing facility
      • Includes scheduling of TCM appointments in office
    • Order Durable Medical Equipment (DMEs) for patients per provider request (wheelchairs, walkers, etc.)
      • track progress of orders to ensure patient needs are facilitated as designated Point of Contact for local DME companies
    • Support for patient care events, such as the practice’s health walks for patients or health education programming
    • Fill in for front-desk shifts as needed: confirming appointments, validating insurance, collecting demographic information of patients, collecting copayments/fees at time of service, answering incoming phone calls from patients and family, and other check in and checkout procedures as needed
    • Additional duties as deemed appropriate to be delegated by the Administrative Coordinator

  1. Clinical Duties (25%)
    • Complete Medicare Annual Wellness Exams (a 60-minute evaluation with the patient, including a depression screening, TUG test, cognitive evaluation, and a care management assessment)
    • Conducts patient chart reviews and analyzes patient health care data to identify and eliminate gaps in care
    • Assist providers in educating and coordinating patient care needs through written and/or oral communication at appropriate level of specificity
    • Back-up for clinical workflow (take vitals, blood pressure, temperature, weight, EKG, spirometry, glucose check, etc.)
